To meet this need, AHHC was established to bring skilled nursing and quality home care services to residents of the Southeast Michigan area. Care is coordinated by skilled and compassionate nurses with a wide range of expertise.

Services Provided
Skilled Nursing The nurses coordinate care, monitor medical conditions and provide education about medication and disease processes. They oversee the management of illness, obtain lab work, provide wound care, manage and instruct regarding ostomies, catheters and other special procedures. The nurse keeps the physician informed about the patient's progress. Physical Therapist The physical therapist develops a home exercise program based on the patient's individual medical needs in accordance with the physician's orders. The goal of treatment is to facilitate a return to independence. Occupational Therapist The occupational therapist helps people improve their ability to perform tasks of daily living, thereby encouraging independence. Speech Therapist Speech therapists, assess, diagnose, treat, and help to prevent disorders related to speech, language, cognitive-communication, voice, swallowing, and fluency. Home Health Aides Personal care services, such as bathing and grooming are provided by the Home Health Aide under the direction of the registered nurse or therapist for those patients receiving skilled services. Social Workers & Dietitians The social worker will assist with long-term planning, and identify available community resources to meet patients needs, in addition, they assist in identifying social factors impeding recovery and help the patient develop coping mechanisms to assist in rehabilitation. How to obtain our services?
A physician determines that the patient needs home health care services and sets up a plan to meet patient's needs.
When a patient is in need of home health care services, the physician and home care professionals establish a treatment plan for the patient. Home health staff will provide a periodic report to the physician allowing for reassessment of the treatment plan.
